Note:
1. Subjected work permit is valid only for 08 (Eight) Hours only an extension of this Particular work permit would be valid Only for 4.5 (Four Hours and Thirty minutes) hours in exceptional cases.
2. Only a few exceptional cases can be accepted with prior approvals of GWS/M&E in charge in case of any emergency & a half-an-hour rest period is to be provided before extended time begins.

Please Note following :
1. Validity of the Work Permit should not be more than 01 day In any case.
2. *Mark fields are mandatory.
3. A Fire Watch should be posted If -
a. Combustible materials cannot be relocated at least 35 ft(11 m) in all directions from the Hot work site (NFPA requirements).
b. Combustible materials are adjacent to the Opposite sale of partitions. walls. ceilings or roots and are Likely to be ignited.
c. It is deemed necessary by the Permit Authorizing Individual.
4. A Fire Watch is to be posted. during hot work operations and for 30 minutes after work has been completed.
5. Medical Fitness certificate issued by a certified and competent Doctor (Min MBBS) is required for all Workers working In confined spaces & at height.
SPECIAL INSTRUCTIONS
A - Mandatory for work at Height/confined space
1. Appropriate PPE is being worn as required (See above)
2. Height Should be more than 07 feet i.e. 02 Meters.
3. Ensure an adequate supply of fresh air is available in confined areas.
4. Take precautions to protect the safety belt must be hung at a good level.
5. Ensure all persons involved are suitable. medically fit and have been adequately trained.
6. Ladder should be the appropriate size and In good working condition.
7. Check the size of the entrance to ensure safe access - take account of the required PPE.
8. Ensure adequate lighting in confined space - provide illumination if needed (non-flammable).
9. Procedure in place for emergency situations.
10. Ensure effective communications between persons inside and outside confined spaces.
B - Mandatory for Hot work
1.Fire extinguishers to be readily available & persons trained In use.
2. Isolation of local smoke detection.
3. Isolation of energized system.
4. LOTO tags should be used.
5. Ensure hot work equipment is suitable for use and in good order.
6. Check location and means of raising alarm.
7. No cutting or welding in presence of explosive material/gases (Remove any flammable containers).
8. Ensure no combustibles (including combustible material in floor gaps) - inspect nearby areas.
9. Suitable protection against sparks and hot particles (e.g. welding curtains) i.e. Any openings or cracks In the walls, floors, or ducts that are potential travel passages for sparks, heat, and flames have been covered.
10. Flammable materials to be removed or if not practicable covered wills anti-flammable material.
11. Cordon off the working area with controlled access.
12. All floors and surfaces nearby the Hot Work area have been swept free of combustible dust or debris.
13. Work area to be cleaned after completion of work (all smoldering residues to be removed).
14. Follow-up inspection after completion of work to ensure no smoldering remains.
C - Others (If any) Please Specify :
GENERAL SAFETY RULES
1. Smoking is strictly prohibited inside the premises
2. It Is your duty and responsibility to detect any hazard that you detect any Hazards should be reported to your Supervisor and corrected by you If you are able to do so else report to HCL Admin team.
3. Observe and obey safety signs: Wet floor, Hazardous chemicals. High Voltage. Toxic Gas, Inflammable substances, etc.
4. Always keep away from restricted areas like diesel storage areas. confined spaces etc.
5. Use handrails when on staircases to prevent falls.
6. Clean up spills to prevent slip and fail Injuries.
7. Avoid wet mapped areas. After cleaning up spills from the floor be sure to mark the area with a wet floor sign.
8. Good housekeeping is important to HCL. Vendors must keep their work neat, clean, and orderly.
a. Clear all waste material daily.
b. Keep passageways and aisles clear.
c. Store all equipment and tools in an orderly manner.
d. Cover or barricade floor openings to prevent trips and falls.
e. Wipe and dry wet surfaces.
f. Do not store spare parts in the work areas.
g. Food is only to be consumed at the cafeteria or at a designated location.
h. Food waste to be disposed of in the appropriate waste bins.
9. Lifting and Carrying:
a. Lift heavy loads with the help of lifting equipment.
b. Don't block your vision while carrying the material Watch where you are going.
c. Before you handle any material, check for nails. splinters, rough objects that may injure you.
d. Never bend your back to carry the load.
e. Bend your knees that are comfortable to get a firm grip on the object and use your leg muscle to lift the object.
f. Avoid Twisting Turn the whole body including the feet to change the direction of movement
10. Prior to maintenance or service, the Vendor should check for the following:
a. location of emergency fire extinguisher, manual call point. Emergency assembly points etc.
b. Location of Emergency OFF buttons.
c. Location and function of safety Interlocks.
11. Follow Risk assessment Hazard Control measures.
12. Recognize the sound of the emergency alarm. Use the emergency exit close to your work area and reach the assembly point. Leave the assembly area only when the All Clear signal is sounded. Do not use the Lift during an emergency.
13. Wear appropriate personal protective equipment for the job assigned.
14. No loose clothing and jewelry while working around machinery with exposed moving parts.
15. Follow the procedures for lockout/Tagout while repairing / Servicing the machinery.
16. Use the right tool for the job. Always Inspect the tool before use.
17. For any medical help approach your supervisor immediately.
18. Do not Indulge in horseplay.
General Instructions
1. Don't roam here and there inside premises.
2. Don't touch un-necessary things while working HCL premises.
3. Don't use Liquor , Cigarette, beedi, Gutkha or any type of tobacco product inside premises
4. While doing any type of hot work like welding /fire related work etc security team must be informed and appropriate Fire Extinguishers should be available at work place.
5. In-case of any emergency situation please call at contact numbers available on work permit or inform to Security Team.
6. Appropriate Personal Protective ladders should be used if required.
7. Use safety belts and other Personal Protective Equipments (PPEs) for any type of work at height more than 2 Meters.
8. Electrical equipments like welding machine /hand grinder etc. should be used under safe conditions only Earthing.
9. For any kind of electrical connections please contact M&E department.Don't try to Connect anything Own self.
10. Don't use wear off old/poor electrical wires while working on electrical system.Use electrical tape on cable/Wire joints.
